Debería funcionar con poner:

$destination_path = getcwd().DIRECTORY_SEPARATOR . 'img' . DIRECTORY_SEPARATOR;
 

o simplemente:

Código php:

$destination_path = './img/';
 

Si solo usas /img entonces estas usando una ruta absoluta.
