creo que al htmlentities le falta que le indiques las opciones ENT_QUOTES | ENT_IGNORE, "UTF-8"
$cadena=htmlentities($cadena,ENT_QUOTES | ENT_IGNORE, "UTF-8")
lo del utf8 es opcional. |
