problemas con pop up

buenas tardes el problema que yo tengo es el siguiente necesito abrir un pop up desde otro pop up bueno el problema es cuando yo habro el segundo pop up ahi no me sale el formulario que necesito espero me puedan ayudar este es el codigo.

Hola:

No sé con exactitud el problema, pero no pongas un onclick en un tag option... cámbialo por onchange en el tag select... pero sobre todo explica mejor tu problema.
